For WorldVistA to work with you, you should discuss what you are going to
purchase before you purchase it so you get the correct file from the AMA.

The WorldVistA kit was created with the cooperation of the AMA and VA after a
lot of negotiating and the signing of an agreement with the AMA. You are
welcome to go this alone, but it took years of negotiations to get this worked
out, so you might want to take advantage of the work WorldVistA has done.

During the VOE project, there was a version of the CPT codes sold by the AMA
specifically for VistA, but once the project was over, the AMA refused to
provide the files in the correct format any longer which is why we went to a
lot of trouble to make this available for the community.

WorldVistA does have a kit that you can use to install CPT codes into VistA
for a nominal charge, but you must have an extant license with the AMA to
use it.

FOIA Release Note on CPT codes

According to the FOIA release notes from Nov 2011, you need to purchase from the CPT codes from American Medical Association (AMA).  The following are instruction from the release note:

"To install the CPT codes (these may be purchased from the AMA on electronic media) for use within VistA, the instruction manual for the conversion and routines may be found on the following link: "https://downloads.va.gov/files/FOIA/Software/VistA_FOIA_Historical_Files/VISTA_FOIA_RELEASES_BEFORE_2008/ICPT-CPT-HCPCS%20Codes/PROGRAMS/"

Upon examination of the documentation - DRAFT Cpt update process guide for software developers, dated 1998, you will find that the instruction for installing CPT code from a tape device.  I did not see any instruction for loading the CPT codes from your local hard drive.  I will check with VA to see if there is an updated documentation that allows installation of CPT codes that was downloaded from AMA online web site.
