popHealth on Docker

I revised the popHealth Dockerfile to use the official Docker ruby container as the starting point, and got it to build. The Dockerfile I used is attached. 

The instance it builds is not useful for development yet because you can't ssh into it. Also, things that should be owned by the pophealth user are installed in root. It would take some more work to get it right. 

New Release of popHealth® Certified to 2015 Certification Criteria

i.    Altering measure bundle – legal issues with duplicating/deriving other measures is unclear

ii.   Requesting new measure bundle with 90 day period from CMS – would take a long time to process
