RE: [EXTERNAL] [architecture] Mapping VistA data to FHIR within HealthShare






I am no longer part of this effort, but yes, the team I was part of was working for the VDIF effort. As of a couple months ago, FHIR DSTU2 was supported for read-only veteran data, but some custom mappings were in progress for data that
InterSystems didn’t have mapped to FHIR resources by default in HealthShare, which is behind the VDIF platform. I don’t know where they are at with that at this point, but there is a major application being built for 2019 release that is relying on this, so
I imagine efforts are moving along as quickly as possible on that.

We have just begun efforts to map VistA data to FHIR within HealthShare, and I am part of the team heading up those efforts. I am not aware of a mapping database – is there someone I need to talk to, Peter? We would like to make sure of
any previous work that has been done but sometimes things get siloed such that one group in the VA doesn’t know what some other group had already done.

VA has been developing VistA to InterSystems HealthShare mapping for over a year now.  InterSystems HealthShare has a FHIR interface.  So if you are in the VA, you should be able to get a copy of the mapping database.

Do you know who at VA this leading the mapping effort?  OSEHRA will be setting up a new group - FHIR on VistA next month that plans to develop an open source FHIR v4 interface for VistA.  It would be great if we can get VA to share this information, i.e., technical documentation with the open source community.  We plan to implement the full Argonaut profile.

I understand that the VA API’s recently announced are to the CDW.  I have always been curious how API’s to the CDW “relational” store compares to the potential API’s directly to VistA (hierarchical)?  Particularly since  data gets to CDW using Cache.  Appears there is significant dependency on “Intersystems” either way?  Which is fine.  My questions are routed in timeliness, performance, and data integrity.  If we are looking at patient centric technology for Care Coordination, these factors matter.
